It’s time to Enroll
During a tour, you will be provided with a complete enrollment package. The forms within the package need to be completed on or before your first day of attendance. For your convenience, we are providing you links to the forms that are most requested and are required for all of our locations. Please note that these links do not contain all of the required forms. Each center may need additional specific forms unique to that location. Your Director will be happy to assist you with the enrollment process.

Please download, print and take this form to your child’s physician. Your child’s physician will need to sign the bottom of page 2. Return this completed form to your centers Director. An annual physical is required by the center and your centers Director will remind you approximately one month before it is due. - Child Information Record
This form is required on or before the first day of enrollment. Please download, print and bring this completed form with you at the time of registration.
